New Client Health Evaluation (or if it's been 24+ months)

This comprehensive 2–4 hour appointment is designed to uncover the root causes of your health concerns—especially when nothing else seems to explain why you’re feeling sick, stuck, or out of balance. Whether this is your first visit or your last appointment was over two years ago, we’ll take the time to truly investigate what’s going on inside your body.

We’ll begin with an in-depth review of your health history, symptoms, and lifestyle, followed by a cutting-edge Zyto scan using FDA-cleared technology. The Zyto system uses Galvanic Skin Response (GSR)—the same proven tech used in polygraph testing—to identify hidden stressors affecting your organs, glands, and systems in real time. Through a simple hand cradle, we can detect imbalances related to:

• Bacteria, viruses, parasites

• Heavy metals and environmental chemicals

• Endocrine disruptors

• Mold and fungal exposure

• Food sensitivities

• Waterborne toxins and more

This data gives us a clear map of your body's priorities, allowing us to create a strategic wellness plan that supports your immune system, detox pathways, and whole-body vitality—without relying on invasive procedures or guesswork.

I also examine visible health clues through your skin, tongue, and fingernails. If you plan to wear nail polish, please send a clear photo of your bare nails the next time you remove it—these subtle details can be deeply revealing.

Full payment is expected at the appointment. I accept cash, check, Apple Pay (no fees); Zelle (2% fee); credit/debit/HSA cards and Venmo (3% fee).

While I do not bill insurance, I can provide detailed receipts for possible reimbursement. Many HSA and FSA cards are accepted, depending on your plan.

Please download and complete the Intake and Authorization Forms from the menu tab or link below. Return them via email (for remote appointments) or bring them with you in person. These forms help uncover patterns and symptoms that might seem unrelated—but often hold the key to your healing.

Appointments are available in-office or remotely via Zoom. If you choose a remote appointment, please:

• Note it in the scheduling form

• Include your full mailing address

• A $250 refundable deposit is required before I ship the Zyto hand cradle

• You are responsible for return shipping

• Instructions for Zyto Remote software and Zoom will be included in the package
